• We help you find healthcare providers who offer elective procedures in your area.
• We are NOT a healthcare provider, medical device dealer, or broker.
• We are NOT a HIPAA-covered entity or business associate.
• We do NOT provide medical advice of any kind.

How the Service Works

• You submit a form with your procedure interest and contact information.
• We match you with healthcare providers in your area who offer that procedure.
• Those matched providers may contact you directly.
• You are under no obligation to proceed with any provider — it's entirely your choice.

Health Data Consent

When you use our matching service:

• By submitting our form, you consent to the collection and sharing of your health procedure interest with matched providers.
• You may withdraw your consent at any time by emailing privacy@electivecareguide.com.
• After withdrawal, we will stop sharing your data and delete it within 30 days.

Provider Relationships

We connect you with providers, but we want to be clear about what we do and don't do:

• We do NOT verify provider credentials, outcomes, or pricing.
• A provider appearing on our platform does not mean we endorse them.
• All medical decisions should be made with qualified healthcare professionals.
• We do NOT guarantee provider availability, pricing, or quality of care.

⚠ Important: Do NOT Send Us Protected Health Information (PHI)

This is important. Our forms are designed to collect your procedure interest — not your medical records.

• Do NOT include medical records, test results, diagnoses, or treatment history in any form you submit to us.
• We are not equipped to handle Protected Health Information (PHI) securely.
• If you accidentally submit PHI, contact us immediately at privacy@electivecareguide.com and we will delete it.

Limitation of Liability

ElectiveCareGuide is not liable for any damages arising from your use of this site, any provider interactions, or any transactions you enter into with providers. Our maximum liability to you is limited to the amount you paid us — which, for consumers using our free matching service, is $0.
